There are two types. In the "ITIL Category" question, the administrator filters categories by type (request, incident, problem or change) and ticks exactly which ones the requester will be able to choose, in a clean list that shows only the category name. In the "CPF / CNPJ" question, for Brazilian tax IDs, the field formats the document as it is typed, checks the check digits, rejects invalid documents and accepts the new alphanumeric CNPJ from the Brazilian Federal Revenue.

The category can come preselected, and conditional questions react to it straight away. The list follows the form's entity and, in the self-service portal, hides categories that are not visible in the helpdesk. The chosen category is applied to the ticket, change or problem created. Exporting and importing the form to another GLPI keeps the categories, and Changes and Problems opened through the form can be created visible to sub-entities.
